Delta 6KVA UPS - I/P Fuse broken

Hi Guys 

We have a few hundred Delta UPS single phase units in stalled. In the last 2 year we have 36 unit coming back with I/P fuse broken. 

The suppliers is not to clear on what might have caused it. 

Any one came across a fault like this?
